Not "my site" — the full domain, including whether it is the www version or a subdomain. Accounts often hold several.
The steps that led to the problem, in order. "It broke" is a symptom; "it broke after I updated the theme" is a starting point.
Sometimes the behaviour is correct and the expectation was different. Knowing both tells us which one to address.
A screenshot or a copy-paste of the exact message. A paraphrase loses the part that identifies the cause.
Roughly when it began and whether it is constant or intermittent. That narrows the logs from days to minutes.
So we do not repeat it. This alone often saves a full round trip.
If you are not sure what is relevant, send too much rather than too little. We would far rather read a long ticket than ask four questions to get to the same place.
